Enrollment of Minor Children in Family Plan. As part of Your Family Plan membership, You are entitled to enroll up to four (4) minor children for whom you are the legal parent or guardian by providing the required information and/or documentation including, but not limited to, a valid Social Security number and date of birth for each of Your minor children.
